HOPWA CASE MANAGER


Job Title: HOPWA Case Manager (Housing Opportunities for Persons With AIDS)

Reports To: President of the Board, Unity Housing Board

Status: Part-time, $650/month, 25-30 hours per month

Location: Hybrid/Remote within the TriCities area of East Tennessee

Position Summary:
The HOPWA Case Manager is responsible for providing comprehensive, client-centered case management services to individuals living with HIV/AIDS who are experiencing homelessness or housing instability. This role focuses on assisting clients in obtaining and maintaining safe, stable housing and connecting them to essential supportive services, in accordance with the requirements of the HOPWA grant. This position requires strong assessment and planning skills, empathy, excellent communication, meticulous record-keeping, and a passion for advocating for and supporting vulnerable individuals.

Essential Duties and Responsibilities:

  • Housing Assistance & Placement:
  • Assist clients in searching for, applying for, and securing appropriate, affordable housing.
  • Provide support with understanding leases, communicating with landlords, and accessing rental assistance if applicable.
  • Collaborate with clients to develop individualized housing stability plans that address their specific goals and needs.
  • Supportive Service Coordination:
  • Connect clients with necessary supportive services, including medical care, mental health services, substance use treatment, benefits enrollment (e.g., SSI/SSDI, SNAP), employment resources, and transportation.
  • Maintain communication and collaboration with service providers to ensure coordinated care.
  • Client Support & Advocacy:
  • Provide ongoing emotional support, direct clients to appropriate crisis intervention, and problem-solving assistance to clients.
  • Advocate on behalf of clients with landlords, service providers, and other entities as needed.
  • Empower clients to advocate for themselves.
  • Documentation & Compliance:
  • Maintain accurate, thorough, and confidential client records, case notes, and service documentation in compliance with agency policies, ethical standards, HIPAA, and HOPWA grant requirements.
  • Assist with data collection and reporting as required by the grant.

Qualifications:

  • Bachelor's degree or relevant experience.
  • Preferred 1 year of experience in case management, preferably working with individuals experiencing homelessness, living with HIV/AIDS, or facing complex health/social challenges.
  • Strong interviewing, assessment, and service planning skills.
  • Excellent interpersonal, communication (written and verbal), and active listening skills.
  • Ability to work independently and as part of a collaborative team.
  • Strong organizational skills and attention to detail for documentation and compliance.
  • Valid driver's license and reliable transportation is required.
  • Ability to maintain strict confidentiality.

Position Overview:
We are seeking a creative, enthusiastic, and skilled Social Media Manager to enhance our online presence. This individual will play a key role in developing and executing our social media strategy, directly contributing to our mission by amplifying our message and fostering engagement with our community. 

Key Responsibilities:

  • Content Creation: Work closely with board member and Sharprism owner, Brandon Davis, to create engaging, relevant, and high-quality Unity Housing branded content tailored for various social media platforms (e.g., Facebook, Instagram) 
  • Content Management & Scheduling: Manage the social media content calendar, ensuring consistent posting schedules and timely content delivery across all active channels at least three times a week.
  • Community Engagement: Monitor social media channels for comments, messages, and mentions; actively engage with our online community by responding promptly and appropriately.

The Community Outreach & Fundraising Coordinator is responsible for developing and implementing strategies to enhance the organization's presence in the community and lead fundraising efforts. This role involves actively engaging potential supporters, coordinating events, and helping grow the organization’s donor base. This position requires strong communication skills, initiative, and a passion for community engagement and development.

Essential Duties and Responsibilities:

  • Community Outreach & Awareness
  • Identify and attend relevant community events for outreach opportunities (e.g., festivals, fairs, meetings).
  • Coordinate speaking and educational opportunities about our work to community groups, schools, or businesses.
  • Organize information tables ("tabling") at events.
  • Discuss the ongoing need for Medical Respite with potential donors, community partners, and clients.
  • Build relationships with community partners and stakeholders.
  • Fundraising & Donor Relations
  • Serve as the coordinating chair for fundraising initiatives and events.
  • Plan, organize, and help execute fundraising events.
  • Identify, research, and cultivate relationships with potential new individual, corporate, or foundation donors.
  • Assist in developing fundraising materials.
  • Commitment:
  • This is a part-time role requiring approximately 10-15 hours a month commitment, with flexibility needed for evening or weekend events.
  • As Unity Housing continues to develop and grow, bonuses and increases in stipends will be considered to compensate for commitment and work.

OPEN BOARD MEMBER ROLES


Unity Housing (UH) is seeking passionate and dedicated individuals to join our working Board of Directors and help end homelessness in Johnson City. Unity Housing Webpage Development Internship: Unity Housing is seeking a passionate and skilled Webpage Development Intern to help us enhance our online presence and better connect with the community. This is an exciting opportunity to contribute directly to our mission and gain valuable real-world experience in website development for a growing non-profit organization. As a Webpage Development Intern, you will play a crucial role in designing, developing, and maintaining Unity Housing's website, working closely with our team to implement new features, improve user experience, and ensure our website effectively communicates our mission and impact. Responsibilities include assisting in the design and development of new web pages and features, maintaining and updating existing website content, troubleshooting and resolving website issues, implementing SEO best practices, and collaborating to align the website with our communications strategy.
